1. Click on “Booking Request Form” to download. Then print it off, complete all sections including the bottle code(s) (if you have lost your bottle with the codes you can send your receipt instead!) Please check the Terms and Conditions out before completing your form!
  2. Then send the form along with a stamped self-addressed envelope  to: Take Off With YAZOO, PO Box 468, Swansea SA1 1RH. All applications must be received by 18/02/11 and all trips booked must end by 18/04/11.

  4. Can't find your code on your YAZOO Bottle? Look here on your bottle, and write the code, number and time as they appear on the second line on bottle (e.g. L630 0734 151337). Simple!
